Do scanned PDFs work?

No — the tool extracts the embedded text layer. Scanned documents (pure images) have no text layer and produce empty output; use OCR software first.

Is the layout preserved?

Text is extracted in reading order page by page. Complex multi-column layouts may read sequentially rather than by column.
